What is the best email form to use for WP?

On my previous contact page, https://www.maryelise.com/contact.htm I used a flash based/php email form setup. I wanted to bring that into my wordpress contact page here, https://maryelise.com/blog/?page_id=141 but not sure how. I tried bringing the php and .swf file into my revolution-mag folder and embedding the flash video source code into my wp html document, but it didn’t work.

I need a form that my readers can use to contact me, and ideally a checkbox for newsletter sign-up. What is the best way to do this?
