When adding items to the cart, they show as FREE instead of the prices specified

Hello –

I set up 2 add-on items which we offer on every sale. You can see them here:

No matter what we do, they go into the cart as $0. They are specifically set to $4.75 and $10.

Additionally, the plugin settings say “type option name if you want already checked”, yet we have nothing in that field and BOTH are checked on our site automatically. The opposite of what the plugin instructions say.
