When are password emails sent?

I’ve just installed this plugin, and I like it quite a bit, but in testing it, I can’t tell at what point an email is generated to give a new user their password. I registered on my site with a different email as a test, and never received an email, even after I approved myself. Some questions that aren’t answered in the documentation:

• Is the user sent a password before they are approved? (as normally is the case)
• Since the password is re-generated after they are approved, are they sent another email with this new password, and is this the first email they receive?
• How many emails total does the registration process generate?
