when click submit form returns missed field error

    The Form on the url suddenly stopped working. It stopped sending out emails. So I updated all plugins, theme, and wordpress. Now form when you hit submit, the form gives an error of a missed field even though all fields are filled.

    After some trouble shooting, the issues seems to be the date field. When the date field is set to required. The form does recognize the text in the field.
