Where is the "unique form id"??

Where do I find my unique form id? Without it, the plugin is useless.
All you have is ‘entries and settings’ options.
God knows what the ‘entries’ means.
PLEASE INCLUDE DOCUMENTATION WITH THE PLUGIN THAT MAKES SENSE.
