WHMCS Bridge Page Not Loading Properly

Hey folks… take a look at my page.

I’ve tried all combinations of settings in the plugin settings area but can’t get this page to load properly. I have the page template set using “WHMCS”.

Using the theme from Themeforest called IWThemes “MegaHost”
