Why is index.php excluded?

Thank you for your inquiry and I am happy to answer.
It’s not the front page that is excluded but the file structure.
In Performance>Page Cache>General, there is an option “Cache front page” so you should enable that option to make sure that the front page is cached.

I’m a bit confused about the answer though, isn’t index.php the front page?
At least on my site, https://www.my-site.com is effectively serving https://www.my-site.com/index.php

It’s not related to that. And you can be sure that this is not preventing your front page from being cached.
YOu should just enable the mentioned settings to make sure that the front page is cached.
