Widget classes and id's go missing

Very strange behavior.
I am running a multisite multi network. When in my development area the widget appears like so.
https://ms.fmcna.org/fmcusa-sandbox-colors/forms/Once the site is pushed live it appears like so. Notice that the Widget title is not styled.
https://fmcusa.org/hr/resources-2/ Looking at the code it does not seem to have either a class or an ID.

Same theme, no different when plugins are on or off. Site wide plugins are:Google Analytics, Multisite Plugin Manager, Networks for WordPress, Simple Sitemaps for Multisite, Wp Curator, WPMU DEV Update Notifications.
