Widget has no email address field

    Hi,

    Try to delete the fields and add them again in the Custom Form Section under General tab.

    This is also happening to me and i can get it to work. The lightbox displays fine, but there is no form in it. Its there on the preview but not on the live thing. The form doesnt seem to make much sense. There isnt any option that would signal that its being hidden
