Wired Permalink, Title Error

Hello,
I’m suffering from an unusual error on my WordPress site. It all began earlier last month where Google started to report 404 errors. Google somehow discovered a link to each post with random numbers at the end of url. Like below
https://tjosm.com/cheap-vps-service-prosper202/1346118223000/
At first above link gave a 404 error. But now it’s accessible. If you check page source, you’ll notice that the page title is “Best and Cheap VPS Hosting for Prosper202 – tjosm.com – Page 1346118223000”
Title should be “Best and Cheap VPS Hosting for Prosper202 – tjosm.com” and it is on following url,
https://tjosm.com/cheap-vps-service-prosper202/
where ” – tjosm.com” part added automatically by SEO Ultimate plugin. I tried deactivating SEO Ultimate and deleting cache. It didn’t fix the error.
So it’s basically same content under 2 urls with 2 titles and 2 descriptions. I’m really worried.
And it got worst recently. These’s already two urls for each post. Google now began to report another 404 error. below is an example,

This page gives an 404 error. Not sure for how long. I’m using Permalink finder plugin. So it’s getting redirected to original url.
I’m lost here, not sure what to next. looking for help
