I needed to create a form for a personal WordPress site and came across Ninja Forms. It is super-simple to install, and use. I had a form up and running in minutes. I was able to tweak the views, add fields, etc. I had 2 questions, which the devs answered quickly. Both were problems with my theme, yet they still supported me! When I need to customize my forms more, I’m more than willing to purchase their extensions. A+
