WooCoomerce Shipping is randomly not being added

I have a client who uses the WooCommerce standard shipping set up with zones and classes. It’s been running fine for almost a year.
Over the last 2 months a handful of international orders have not had any shipping applied to them. I am UK and the affected orders have been in Europe and US which are 2 different shipping zones. I’ve checked the zones, classes, that the products have the right shipping set up, coupons etc everywhere I can think.
I’ve set up a staging site to mimic the orders and I cannot recreate the issue at any point.
Could anyone point me in the next direction of where I can be looking to try and see where the issue may be?
