WordPress Contact Form in index.php

  • I created a site now turning it into a theme. I need to create form like this on our website.

    Would rather create it from scratch myself. Do I just need a form and then the action would be a file inside the theme folder that uses wp_mail?

    If anyone thinks they have a better way please tell me. I am keen on doing it myself because I know exactly what stuff I need but would love to hear ideas or confirmations on how to go about doing it from scratch. ??
