wordpress download monitor plugin help

I love the wordpress download monitor plugin but the author no longer supports it. I cannot find how to make the original download page that it shows in screenshot 3. That shows all your downloads I can create my own with codes but would have to updae it every time I add a download.

I was wondering if someone that might use it already might have the answer. The instructions were out there a few weeks back. But was just trying tofigure out how to make the downloads work at the time. Now the instructions are gone only a readme file I can find with no explanations.
