In includes/admin.php on line 438 you directly include the media upload javascript using
wp_enqueue_script('media-upload');. This breaks WP3.5 beta 3. Whilst I expect it will be fixed for the actual release I have replaced it withwp_enqueue_media();which fixes it. See this core trac ticket for more details.https://www.ads-software.com/extend/plugins/wp-document-revisions/
