WP admin bar disappeared

For further testing, can you first please make sure that you are using the latest version of our plugin and then remove the form from our pop-up and test again to see if the admin bar is visible.

I am experiencing the same issue right now, too. Boxzilla is wrapping #wpadminbar inside the div that has display: none; set inline. Happy to share testing information / staging site privately!
