    I activated WP Smush and bulk smushed about 30 images per domain. ON the domains that I installed WP Smush the images immediately began to disappear… both on the site and the media library. By 72 hours, they were all ghost icons, but the images were still there somewhere… just no longer visible in the media library and definitely not available for the site. Other icons, Facebook, Twitter, etc. were included.

    Tested for virus and hack. No. Optimized with SQL queries several times to no avail. Samantha has given you the affected domains.

    When I’m trying to access your site, I’m getting a “Your access to this site has been limited” message by WordFence.

    Can you possibly deactivate WordFence and check if it fixes the issue.

    Have you installed any other updates recently?

    @livingrelaxed, I just deactivated Smush on your site and tried uploading a image.

    The image doesn’t appear, which means the issue is mostly related to your Web Server.

    You are using Apache, Check for a recent changes in your .htaccess files made by either you or some plugin.

    Also, to add to the above reasoning, image from your plugin folder aren’t loading as well, which are not affected by Smush in any way.

    I appreciate your suggestions Umesh. Although, as stated initially, ALL plugins, including WordFence, had been deactivated in an attempt to solve the problem.

    Additionally, I have had many updates to the .htaccess files which have been made by WordFence long before Smush was recently added.

    @livingrelaxed,

    Try restoring .htaccess to Pre Wordfence installation, if you’ve any backup.

    In past we had similar reports, but in the end it comes down to some other conflict or server issue.

    Hence suggesting you to take a look at .htaccess
