wp-user-frontend attachment options not dispalyed

I activate the plugin wp-user-frontend in that admin bar i enable the attachment still i call the plugin by shortcode [wpuf_addpost]
only the tilte with description and the attachment name only displayed not the (upload box) anyone help please………..and i need to upload my videos in the post by front end any one…..https://www.ads-software.com/extend/plugins/wp-user-frontend/
