[WPCR_INSER] short code not work proper.

i have use wordpress 3.4.2 and wp customer review 2.4.5 and i call plugin short code in template.. customer review form show but form not submit.. anyone can tell me is customer review version 2.4.5 work with wordpress 3.4.2 or not through shortcode.?
