    Dear Support,

    I’m embed the wpform to homepage, a simple inquiry form, but at the below of the question, coming out another extra field that i’m not request it at all.

    may advise how to fix it? screenshot here and here.

    I tried to open the screenshot link you’ve shared, however, getting request access message and I’m required to sign in. When you have a moment, can you please remove the request access permission or share the screenshot via file sharing applications so we can take a closer look?
