WPForms Contact Form Disappeared SKT FSE

    I use WPForms to provide a contact form on my WP site (site theme: SKT FSE). All worked well for about a month. Now the contact form has disappeared from the site (at an unfortunately critical time). Troubleshooting mode indicates that the theme is the problem (form works when the them is deactivated). Any assistance would be appreciated!
