Yes/No Checkbox

I have a checkbox that I would like to have return the value “Yes” if checked and “No” if unchecked. I’m hoping for a result that will appear on the cart page as something like: “Do you want this extra option?: Yes” or “Do you want this extra option?: No”. So far I’m only able to get the appropriate value (“Yes”) to show if the box is checked, if the box is left unchecked nothing appears on the cart page.

Essentially, I’d like to know if it’s possible to have the checkbox return a value (in this case “No”) if left unchecked.
