Yoast Appending my URL/Permalinks with /#.Uin497wTeBY how do I get it to stop

I have an issue where Yoast SEO is appending my URL. By adding /#.Uin497wTeBY to the end of the URL. How can I stop this from happening?
here is an example of what it is going on:
https://mysite.com/san-francisco/#.Uin597wTeBYand it should be:
https://mysite.com/san-franciscoAny help would be appreciated.
