Your plugin is missing tick and cross

    or change the reference to:

    /wp-content/plugins/gravityforms/images/cross.png
    /wp-content/plugins/gravityforms/images/tick.png

    (anyone who needs a quick fix can also download from there and put in the addon directory image folder)
