Youtube display in form

    I want to create a form that allows a visitor to enter a Youtube ID or URL into a field and in response to the entry displays the video in a second field/player on that form.

    I then want to be able to use that YouTube URL or ID in a survey where other users can review and critique the video contents.

    Is this possible to do with the current form maker or is it a feature that would be considered for a future release?

    Unfortunately, Form Maker does not have this feature, since it is not a standard form builder functionality. Hope this is not a problem for you.
