    this is resolved, it was because the user wasn’t uploading custom thumbnails.

    Forgot to reply to this. There is also the iframe_placeholder option to turn on or off the placeholder image, if you don’t have control over the image. I’m hoping to get another option like placeholder_image but I’d really need to finish the Media UI for generating the shortcode first.
