Same way “Hide down votes completely?” should also not selected.

You should try on your localhost on pc only. Or you may try on live test site if you have any.

The COD will display only on checkout page as it is the payment method.
Yes the extra charges will be display on cart page to inform the buyer about extra charge as per condition set.
