The issue is now apparently solved as it was a conflict with an old Google Adsense plugin injecting code that did not make your excellent mailchimp for wordpress plugin work properly.

first of all thanks a lot for the prompt feedback.
I already tried to copy the shortcode in the page and nothing showed up (i mean the form itself)- as you can now check, i deleted the mark-up code from the page

Hello there,

this is happening to me as well.
The form shows up correctly but on button click nothing happens.
API are ok, i tried to change the various settings (CSS etc.) without success. This is happening when creating a page for the form – on the other hand it does work if i put it in a widget on the main sidebar.
Here’s the page i’m trying to setup.
I’m on WP 4.2.2 using Generate Pro theme (Genesis framework).
I have other Genesis WP websites using Mailchimp for WordPress and they work just fine.
