I also lost access to the back end of the site, courtesy of a server error that has now been fixed. Trying your solution posted above. Will let you know.

I switched from Ninja Forms to Contact Form 7 and I now have it working.

My field mapping is correct.
This sounds like the same issue I’m having. I used Ninja Forms to make the data, but then also tried using a basic .csv file that I uploaded. Neither of them worked to create readable data. When I export the leads and then open the .csv file, it is showing me information about the browser (Chrome) rather than ANY of the lead information that I have imported. I notice that the “Status” is functioning just fine, because I applied it to the blank fields that I selected from the Lead Management Menu.
In fact, here’s what I get in the fields of the exported .csv file:

I also cannot get it to download any of the sample .csv files. It just gives me a blank screen. If I could get one of those to download, then I could try uploading one just to see if it would work on your own .csv files. But I can’t even do that test.

I’m having the same problem.
I’ve imported from Ninja Forms, as well as importing from basic .CSV files, and although it shows the entries are there, the fields are all blank.
When I export the entries as a new .csv and read them, the fields are filled with gibberish about what kind of browser I’m using (Chrome), rather than ANY of the actual customer data. I believe I’ve mapped the fields correctly, but regardless of the scenario, the plugin simply won’t show me usable customer data.
Is there any way we can have a simple new customer input screen so we can just put them in one at a time if we want, rather than having to either upload or use another plugin?
