Con la información que has puesto veo que está todo correcto. Decirte que el plugin sólo funciona en la parte pública de la web cuando estás logueado como Administrador. Te aparece en la barra superior un nuevo menú llamado “Storefront Visual Guide”.
Lo he probado en una instalación de pruebascon las mismas características que has puesto y sí que funciona.
Si te está dando otra clase de error PHP por favor pásalo por el foro y le vuelvo a echar un vistazo ??

Meanwhile let you know that the “template” name is case sensitive.
You wrote in your style.css:
Template: StorefrontAnd it should be:
Template: storefrontBut nevertheless I’ll be fixing it in the plugin.

If you have a similar issue also check if other plugins are disabling menu options.
