In Woocommerce>Settings>General the address is correct. It’s also the same as the one on the other site that is working.

So I went into functions, created a new sidebar then called the sidebar into my index.php.

Then I placed the shortcode into the custom widget in the Admin section and the form shows up.
Only problem now is that it says the information was sent…but I never get the email.

Well I don’t use static pages, but customize my index.php. But I will try adding a custom widget and put it in the location that I wanted it to be.

It was an issue in my header.php

I’m having the same issues.

That fixed it! Thank you so much for being a brain that hadn’t been hunting through code for hours.

I went into the database and in the wp_options, the blog_id # was changed. I changed the number in define ( ‘BP_ROOT_BLOG’, #) in wp-config.php and everything seems to be working great!

I couldn’t get to the network, so it was suggested in some other posts in the forums that I delete them from the server through FTP.

I actually needed to step away and get back to it with a clearer head. Finally got what I need.

So I’ve created the folders and did my best, but still at a loss. What code would show a separate correct and incorrect message based on the quiz score?
