Hi, we are having the same exact issue. Blank settings screen.
The plugin worked fine on our staging build, but once we transferred to client server the settings panel is absent.
The staging build is IP locked, so unfortunately i cannot share the link. However here is some info:
WordPress 4.0
PHP 5.3
Server version: Apache/2.4.10 (Unix)
MySQL Server version: 5.1.73
Looks like this is a common problem as 3 people posting likely equals many silent sufferers ??
Please look into this so we can use this great plug-in. Thanks!
I should mention we also tried disabling ALL plugins and only running this one. Still no settings screen.
