Ah – delete local backups under “Advanced” settings is not checked. I didn’t realize that was even there. Let’s see if that does the trick.

Sure. I think what is happening is that there are glossary terms being used in the form.

Right now, I have Glossary disabled, and the form is fine.

When I have Glossary enabled, all that appears is the text at the top of the page.

I created a staging site that is a replica of this site, and I have glossary turned on there:

You’ll see that the form does not render at all.

Thanks! Everything was set to 755. I have a feeling I messed it up while I was playing with the free version, so I removed and reinstalled both plugins and it appears to be working.
