I have installed Google’s Pagespeed extension for Nginx and I had activated the defer_javascript module which was causing the problem. Now that I’ve disabled it everything seems to be working good.

@jesse – I am running my website on a Linode VPS and I’ve installed Nginx, MySQL, and HHVM for PHP. Perhaps the HHVM is the problem. I am going to try and switch back to PHP-FPM and see if that fixes the issue.
Edit: No it doesn’t.
Edit2: Chrome dev.tools doesn’t give any errors however IE’s dev.tools does contain the following

In reply to: [Clef Two-Factor Authentication] Blank clef-box on wp-admin@jeese – I checked whether I have enabled any extensions in incognito mode and I haven’t. The JS console doesn’t give any errors.

What you suggest will either put the comment in the Moderation Queue or in the spam.
What I would like is to deny access to this spammer so that he cant even leave a comment on my blog

Yes, thanks. This is useful. But I still have the problem when I input an IP in the Better WP Security plugin admin area that I get the message “You cannot ban yourself. Please try again.”
The exact same thing happened to me and I just installed JetPack (first time user) so I naturally thought that I did not setup something correctly.
