Update:
I removed Nextgen Gallery completely. Then, I deactivated all of my plugins. Then I installed Nextgen and started activating one by one. Akismet, Custom Post Type UI, then I activated Advanced Custom Fields and got the white screen again. So it appears to be a conflict between Nextgen and Advanced Custom Fields?

AginityBB, were you able to find a solution? I am in the exact same situation, need to write form data to an existing MS SQL Server database.

OK, nevermind, I figured it out. As usual, I was over-thinking. Adding a class manually through the Menubar interface works a treat. Thanks again.

Thanks for responding Andrea. Would you be able to shed a little more light on exactly how I would go about adding a specific class to the last item in the list? As it is, the code is generating class=””. Unfortunately, I know just enough PHP to get myself in trouble. ??

query_posts('cat=-51');
