I just went through the steps again and this time it appeared successful. I deleted the existing database and user, and then recreated them. Duplicator ran and I have the tables in the DB. When I log into WP it acts as a new install and none of the plugins or themes, etc are there. I guess I should start from scratch and make a new package.
According to the installer-log the database connection failed with Access denied for the user name I have setup on the server.
I attempted to run it in ” Basic Database Only Mode” as I have a lot of large files within WP soI just uploaded them fia FTP. When I selected “cPanel it indicated that the host did not appear to support cPanel. I left all options at the default.
When selecting “Test Database” I get the following:
Host: Fail
Database: Fail
Version: no connection
Compatibility: This Server: [no connection] — Package Server: [5.6.37]
NOTICE: The current version detected was released prior to MySQL 5.5.3 which had a release date of April 8th 2010. WordPress 4.2 included support for utf8mb4 which is only supported in MySQL server 5.5.3+. It is highly recommended to upgrade your version of MySQL server on this server to be more compatible with recent releases of WordPress and avoid issues with install errors.Thank you. That did it.

As I said in my above post. “I have gone through the process of disabling all plugins. When everything is disabled, including UM, the toolbar shows properly. When I enable UM (the only enabled plugin) the toolbar does not show.”
I believe the issue is that the “Sync / update all existing users function is not working.
UM-> User Roles-> Author-> Sync with WordPress Role.I manually went to WP Users-> Selected a user with the “Author” role and manually change his “Community Role” to “Author” and the toolbar shows as it should.
Something is keeping the sync from working. With all plugins disabled, it does not sync.

Lat question unanswered, but it is working with the new structure.
Should this structure have caused an issue with the installation?

Since I was just starting the site, I blew it all away and started over. It seems to be working now.
Initially my setup was: domain.org/folder/wpfilesThinking that maybe the plugin didn’t like the site being set up like that I went with all the defaults so that now it is domain.org/wpfiles
Last evening i uninstalled and re-installed the plugin with no difference in the outcome.
Any ideas?
