Where may I edit width and length of iframe?

And to add no scroll html code.

Not 100% sure what you mean? If you mean the price table is not centered, simply use text tab and place this between shortcode:

There is no submit button or upload button

actually, it is there if you click around a bit, but, you cannot see it.

I cannot seem to get the file upload to show at all?

I have placed the:

Into the form, and also in the File attachments:

I am now using short code of: [wp-members page=”password”]

however had a community profile page / plugin from: wpsymposium

so the https://xxxxxxxx/profile?a=pwdreset goes to my community profiles for members.

So, I changed User Profile Page: to a new page I created for forgotten password.

https://xxxx.forgot-password/ but the Forgot? in login box still takes you to: https://xxxxxxxx/profile?a=pwdreset and not the

New https://xxxx.forgot-password/

I thought this was a cache issue, but I cleared cookies, restarted pc, and do not have a cache plugin.

Any thoughts, or work around?
