That’s easy to achieve using CaptainForm, and there’s multiple ways you can do this.
The easiest one is simply adding Name and Email fields to your quiz, and integrate it with Mailchimp, when they submit the quiz, they also get subscribed to your Mailchimp emailing list.
The second one is creating 2 forms, one where users can subscribe, and a second one which is the quiz.
In your subscription form, go to Settings and then to Notifications, scroll down to the bottom, and at Thank You Page & Redirects, select Redirect to a web page. Once you check that option, an input box will show up underneath where you can type in the URL of your quiz: https://prntscr.com/c3f344
Now people will get sent to your quiz automatically, after subscribing.

If you have the files online, that’s great because we will simply link them to appear on your form. We can arrange the options for your clients on rows, columns and show or hide them depending on conditional logic. You can add headlines or additional text to any field. You can also select multiple options by using checkboxes.
Do you want your clients to be able to upload files of their own?
We allow unlimited form views, so you don’t need to worry about that. Our platform has strong anti-spam features.It seems that your scenario is fairly complex. There is no extensive work for this. The videos are simply embedded within a HTML block. Do you have your samples stored online somewhere?

This would only be possible if you split your form in multiple pages.
Once you did that, you can select the progress a user makes through multiple methods.
One would be showing the page number a user is on and how many pages are left, like so: https://prntscr.com/b7bhde
The other would be showing it as a percentage, where the form will calculate the percentage based on the page the user is on and how many pages are left, like so: https://prntscr.com/b7bhl5

The only issue that we are aware of until now in what the activation is concerned is the one with the third party cookies – the plugin needs to have them enabled in the browser in order to get activated. We are currently working on getting this fixed, so the activation will work either way, third party cookies enabled or not.
In the meantime, please check once again that you have third party cookies enabled (not the regular cookies, but the third party ones) and let’s schedule a screen sharing meeting. Please let us know at [email protected] when it would be a convenient time for you.
