I’ve started another thread that I think is the same issue you are having.
What I’ve found is that unchecking ‘Append account suffix to new created usernames’ (that Vagner suggested) resolved my issues with the bulk import not working. However, when I attempt to log in directly to the subsite where this setting is changed and the import is working (i.e. https://domain.com/subsite), the login fails and I receive an ‘Error creating user!’ message on a white screen.

I finally narrowed down the problematic behavior I’m experiencing on the subsites of my WP Multisite network. Under the ADI settings User tab, in the top section labelled ‘Account Suffix’, the checkbox ‘Append account suffix to new created usernames’ is causing two conflicting problems, depending on whether the box is checked or unchecked.
Unchecked State
When ‘Append account suffix to new created usernames’ is unchecked, I can run the Bulk Import process and all relevant user accounts are imported and/or updated with the correct roles based on the settings under the Authorization tab. However, when a user attempts to log in directly at the subsite (https://domain.com/subsite), the ADI plugin returns a white screen with the message ‘Error creating user!’Checked State

Currently, we have approximately 30 subsites within our multisite install. Obviously, in order to be able to create daily automated tasks to import/update user accounts with the correct roles, we have to leave the ‘Append account suffix to new created usernames’ unchecked, or else the bulk import process will fail. But, leaving this option unchecked results in users receiving an ‘Error creating user!’ message if they attempt to log in directly to any subsite. (Thankfully, they can still log in directly to the root site and from there access the rest of the network.)Can the plugin author or anyone else provide any insight as to what’s going on, or any guidance on how to possibly resolve this conflict?

Closing and re-opening a new thread.
Further testing shows one of three identical installs and configurations is working correctly. In a Dev, Test and Prod environment, Dev is correctly assigning roles to Active Directory groups with the bulk import function; the TEST and PROD environments are not, however.
I have looked at every aspect of the plugin configuration and site configuration between all three and they are identical – all on WP 4.3.1, all ADI plugins are same version, all other plugins are exactly the same and same versions, and enabled the same.
So, at this point, I can’t yet tell if my problem is related to the plugin itself, or if the issue is outside of the plugin.
