One more question: with the pro version: can I just select one folder in my server to scan for images not present in library?

The problem was caused by Wpml Media plugin. Once I fix this I will try the pro version to delete files in my server.
@tigroumeow: when your plugin delete files in the server it touch nothing in the database, can you confirm that to me? Thanks

One more question: Can I create an scheduled task to keep the Media Library and Server folder clean.

If not possible with the free version, can I do it with the premium version?

The issue is that the reviews have been the same for the last 30 days. You say that it should be updated every 14 days but it doesn’t in my site.
How can I trace and fix this?
