Maybe there was some caching problem, when disabled all plugins.

I did not modified page templates. Using child theme for Twenty-twelve, but you can try with default twenty twelve too.

6.) We prefer all files in the defined upload directory and in the thematic subdirectories (created under upload directory.) It’s enough to upload via FTP to thematic subdirectories. We don’t want to store downloadable files in YearMonth sudirectories, but it’s nice when uploading pictures to media manager.
