Ah, I apologize for misunderstanding your inquiry. At this time, the contents of HTML fields would only display in the form editor only when editing that HTML field. I have created a feature request for the ability for text entered within HTML fields to also display within the form editor without needing to edit the HTML field.
Text entered within the Label of the HTML field should not be invisible, however. Does the issue persist with other add-ons momentarily disabled and when using a default WordPress theme, such as Twenty Twenty?

I’m sorry to hear that you are experiencing this. I tried to visit the linked screenshot, but unfortunately, the page required a password.
On my local site, I built a replica form using the description you provided. After testing, I was able to replicate the issue when setting my site’s language to Arabic.
I have created an issue for our development team to investigate so that they can release an update within a future update of Ninja Forms.
Unfortunately, I do have a hard ETA of when the solution for this will be released, but our engineering team will investigate this.

Within the HTML field of your form, you can use the “Label” setting so that you client can identify the HTML fields that need to be updated. The Label of an HTML field would not display on the front-end of the form, so it can be used for backend purposes:

Thank you for reporting this. I was able to replicate the issue of stars over 5 not displaying the additional stars when viewing the Submissions page. I have created an issue so that our engineering team can release a solution for this in a future update of Ninja Forms.
I was unable to replicate the issue of the Star Rating data not being included within the CSV export, however.
Does the issue persist when temporarily disabling other add-ons and momentarily switching to a default WordPress theme?
If you would, please navigate within your site to Ninja Forms > Settings > Advanced Settings. Here, please check the box for “Show Legacy Submissions Page”. This will display the prior, legacy version of the Submissions page as well as data exporter.
When using the legacy page, is the data included in the CSV export?

I’m sorry to hear that you are experiencing this.
The link was password protected and I was unable to view the affected form. With Opinionated Styles disabled, styles from the theme and other plugins would be allowed to apply styles to the form.
May we have a screenshot of the issue? You mentioned that the Radio bubbles were shifted to the right. Were the bubbles still aligned with each other or were they staggered like steps?
If they were staggered, I would try adding CSS to apply a “line-height”.
Ex:
line-height:30px;You would add this in the same place you added the accent color.

Is the WordPress environment using WordPress 6.1.1 and a PHP version of at least PHP 7.4?
Try temporarily deactivating all your plugins that are not for Ninja Forms and switching to a default WordPress theme like Twenty Twenty, then test to see if the issue persists. If this solves your issue, then the problem was caused by a conflict originating from one of your plugins or your theme. Reactivate them one at a time until the issue returns to determine which plugin is causing the conflict.

In Ninja Forms, field merge tags use curly brackets rather than square brackets.
Ex: {field:your_field_key}
If you would, please edit the Email Action of your form and try inserting each field by using the merge tag button:

Thank you for providing us with a screenshot. The field input boxes can be targeted with CSS that target the following elements:
.nf-field-element input, .nf-field-element select, .nf-field-element textarea
Ninja Forms also has a Layout and Styles add-on which can help to style your forms.
One suggestion would be to activate “Opinionated Styles”. This setting can help to enforce certain styles if the site theme and other plugins are altering the look of the form. To enable this, navigate within your site to Ninja Forms > Settings > Advanced Settings. Here, you can change the “Opinionated Styles” setting to “Light” or “Dark” and save the settings.

I’m sorry to hear that you are experiencing this. I copied the example list and imported it within a Select field on my local site, but I was able to view submissions for my form successfully.

The “Health Check and Troubleshooting” plugin can be used in order to test your site without impacting any site visitors. You would be able to disable plugins and switch to a default theme for only your browser session.

You mentioned that you tested with other add-ons disabled previously. If you would, can you reinstall the 3.2.8 version of Ninja Forms? You can download prior versions of Ninja Forms under the “Previous Versions” section at the bottom of the following page:
https://www.ads-software.com/plugins/ninja-forms/advanced/Were your forms built on the prior 2.9.x version of Ninja Forms or were the forms always built on the current Ninja Forms Three version?
If the forms were not converted from the 2.9.x version, this may be related to the issue. If the forms were not converted to Ninja Forms Three, after reinstalling the 3.2.8 version, please navigate in your site to Ninja Forms > Settings > Advanced Setting and a button to “Convert to Three” will be available.
