The easiest solution would be to move all of the contents of the wordpress folder into the public html folder. Other than that, depending on your level of access to the server you can edit the virtualhost file to point to that directory, or create an .htaccess file to rewrite the url. If you are using a shared host though, this may not be possible.

Go to you admin panel, click plugins and add new. Search for WP Survey and Quiz. Click install. Then click activate.

I’ve had a similar problem when images didn’t complete processing (the php script timed out). Which is usually caused by too large a file size. Try reducing the size of the photo, or at least the file size with compression. Just a shot in the dark but it might help.
