You are correct that I was logged-in when the fields did not show, as I discovered and posted a reply. The plug-in is excellent, however as a newbee, it was not clear from the quick start guide that this is how the plug-in responds when using the preview button when creating the event details. Thank you for your reply.

Well this is very interesting. If I use the new link that I posted in Chrome, all does show correctly. Things do not show as I described when I am working in the backend of the website and using the preview selection to see the event. I guess there is not a problem after all.

To clarify my problem, the #_BOOKINGFORM (default single event template setup) is showing the tickets that are available and comment area but it not showing the spaces for the ticket buyer to enter their name, e-Mail and phone number or the login. I have not make any significant changes to the setup other than the date format.
