I have the same problem. The icon selection area only shows the Sharexy icon as well.

Thinking it might be a Firefox browser plugin issue, I tried it in IE. Same issue.

The placement (checkbox) and functionality work fine but no customization of icons or look are possible.

This kind of plugin is for readers to be taken to Their social page to share your post.

Linking to your social sites at a specific address is a separate thing and easier done with some free icons and a little HTML. The third kind are links to your feeds.

It’s much better to offer a URL with your domain than a random short url that people don’t know what they’re clicking on.

In another post, they ask you to email the maker for support.

I have the stated problem and am not on Multisite.

The sample screenshot you have shows it showing After the Submit button.
I just installed it and it comes up before, where it should be.

I found the fix

View, Page Source, then search for Captach to find the CSS plugin name.

Found the fix.

clear:both;
}

Using View Source and searching for Captcha, I found the CSS label.

It apparently does not work with MadMini per someone in Support.

The MailChimp Plugin includes this and for the Contact form.

Same problem over at wp.com
The development environment doesn’t let it connect properly.
I just ran into the same problem myself, accessing the wp-admin via local host file edit.
