I’m glad you were able to figure this out. As for why it may have happened, it’s difficult to say. My suspicion though is that it might have to do with the comma in the variant name “iPhone 12 Mini 5,4 Inch”. Facebook may have been interpreting the comma as a break in values.
Was the variant named exactly like that before? And do you have other variants with a similar name that synced without any problems?

Also, are you trying to view your site from a compatible device with a payment method added? One way you can check for device compatibility, at least for Apple Pay, is by visiting https://applepaydemo.apple.com/
If the device you’re using is compatible, the Apple Pay button should show up like so:

As for the missing events, can you check what information the Pixel Helper gives when you click “Troubleshoot Pixel”?

For example, the AddToCart event may not be tracked if you’re using a plugin to redirect to the cart page after adding a product to the cart. Can you check if you perhaps have any such plugins on your site?

One thing we have not yet checked, but that can often cause the “Oops, something went wrong.” error is the default currency set in your WooCommerce store. From the System Status Report you’ve shared with us, I see that it’s currently set to AUD, which should cause no problems as it’s in Facebook’s list of supported currencies.
However, can you try temporarily switching the currency to USD, CAD, GBP, or EUR just to establish the connection, and then switching back to AUD once that’s done. We’ve seen others find success with this method, so can you see if that will work in your case?

I took a look at your site’s checkout page and didn’t notice any obvious errors from the browser inspector. However, do you recall having made any recent changes/updates to your site before this happened?
The first thing you’d want to check here is the order notes on the right when editing the failed order. Are you seeing any clues from there?
Second, if you have logging enabled under WooCommerce > Settings > Payments > Stripe, you can also check for Stripe logs from when the order failed in WooCommerce > Status > Logs.
Third, also under WooCommerce > Status > Logs, are there any fatal error logs from the date of the failed order?

One thing we have not yet checked, but that can often cause the “Oops, something went wrong.” error is the default currency set in your WooCommerce store. Can you let us know which currency is being used there?
To note, the currency must be in Facebook’s list of supported currencies. You can also try temporarily switching the currency to USD, CAD, GBP, or EUR to establish the connection, and then switching back once that’s done. Can you see if that will work?

Can you let us know more about how you’re making the new checkout page? Are you using a page builder like Elementor for this?
Can you also try if using a simple page with only the
[woocommerce_checkout]

Are you able to reproduce this issue consistently? And can you check what role the customer is being assigned instead? Also, are the customers showing up correctly under WooCommerce > Customers?
Do note that roles will only be assigned to new accounts. Existing users will not have their roles changed.

The error messages you’re getting seem quite uncommon, but I hope we can help point you towards a solution.
Do you perhaps have different Business Managers? Can you please check if you’re initiating the catalog from the same Business Manager that you created it from?

While we’re waiting on a fix, you might want to try the workarounds suggested in this thread: https://www.ads-software.com/support/topic/memory-limit-reached-after-last-update/
Specifically, some users have found success with renaming fb_google_category_to_attribute_mapping.json so it is no longer loaded. Another thing you can try is increasing the amount of RAM which would be available for WordPress to use. From the error message you mentioned, it looks like it’s currently set to 185 MB. I get $1.03 in tax for Colorado Springs on a $19.99 product (5.15%) and $0.74 (3.7%) for Fort Collings. $0.80 (4%) for Denver. Definitely strange.
I’ve tested your settings on my site, and I get similar results with a Fort Collins, CO 80524 address in my store’s General settings and a USD 19.99 product. The thing I noticed though was that, when I used the same zip code 80524 at checkout, I had a tax rate of USD 1.51 (7.55%). Using 80525, however, gave me USD 0.74 (3.7%).
It turns out that collecting Sales Tax in Colorado is a little different from other states. In brief, with your tax nexus in Colorado, you only collect the full combined sales tax rate (e.g., CO 2.9% + Larimer 0.8% + Fort Collins 3.85% = 7.55%) for sales in the same district.
For sales to customers outside your district, you only collect “retailer’s use tax,” which is CO 2.9% + any local district taxes (e.g., 0.8%).
This matches what I saw in my testing with a CO 80524 store. Sales to 80524 were charged the full 7.55%, but sales to 80525 were only charged 3.7% (C0 2.9% + Larimer 0.8%).
