I had a similar problem and found a fix. I hope it works for you, too. Access the Settings page for the plugin. Scroll down and click to display Advanced settings. Scroll down to Headings levels and check boxes for the levels you want displayed in the TOC. The new installation seems to have no boxes checked initially.
