I completely deleted all of the w3tc files, then installed the plugin again. That resolved the issue.

/plugins$ rm -rf w3-total-cache
/plugins$ rm w3tc-wp-loader.php
/wp-content$ rm -rf w3tc-configs
/wp-content$ cd w3tc-config
/wp-content$ wp plugin install w3-total-cache

@ashduram Are you using CMB2 for your custom post types?

The issue is due to the updated Instagram Platform Policy and your account being in sandbox mode.

I was able to get the plugin to work using an already authorized Instagram app, instead of the one I was developing in sandbox mode. Hope this helps!

Same issue. No images are loading. Purchased the premium plugin and it still does not work at all.
