Hi,
I have the opposite problem… ??
When I set the weekly run to be on Monday, the next run is set to Tuesday and so on.
The selected day of the weeks is X and the next run is X+1.
I used the latest version of WP and this plugin.
I hope the fix will solve this one as well.

Also, the log page shows a long matching list at the “all lockouts” box, but on top of the page, in the box of “clean database”, there is a line of “Your database contains 0 old lockouts.” but it should not show “0” (zero) but the number of records of “all lockouts”.
Still, checking its check box and clicking “remove data” removes the current “all lockouts” records, so this works well.

I have similar issue.
I haven’t banned anyone manually, but BWS did so for an IP address after it had too many 404’s, so it locked it out permanently.
Still, the attacker continued on, making more 404’s, which run the 404’s detection process all over again and for each pass, so I get both many notification emails and practically the permanent lockout is not working.

Also, as a solution to both protecting your privacy between domains but enable refer for needed apps in a unified domain – you can add to FF the add-on of “Smart Referer”, https://addons.mozilla.org/en-US/firefox/addon/smart-referer/, which sends this header only within the same domain, but not cross-domains.

OK, found the cause of this, after comparing the headers being sent from both Chrome and FF, and found that the “referrer” header is not being sent to the server by FF, which reminded me that I have disabled it in FF manually.
My FF config field of “Network.http.sendRefererHeader” was set to zero (0). I reverted it back to its default value which is 2, restarted FF and now all is fine.
