Thank you for trying and responding. I was able to ‘resolve’ the issue by avoiding it. Under ‘Form Appearance’ I had to choose a different Form Style. The style that came with a theme I purchased did not work, but when I select a different style (I chose blue), it works. Only problem now is it is not centered for whatever reason.
